The Memorial Committee is responsible for the design and maintenance of a memorial garden where markers remembering loved ones and/or ashes may be placed in a meditative setting on the grounds of the Unitarian Universalist Fellowship at Stony Brook.
You are encouraged to fill out the forms listed below to indicate to our minister your plans for burial and memorial services when needed. You can request these forms from the UUFSB Office by phone (631-751-0297) or by sending an email to office@uufsb.org.

We are grateful for donations in memory of friends and loved ones. If you have expertise in various phases of end-of-life planning, such as advanced directives and/or wills and would be willing to participate in programs sponsored by the Committee or for consultation on an individual basis, please let us know.
